Patients:
If you have questions, please call our Billing Team at (716) 831-2700 Monday through Friday from 8:00AM to 4:00PM. Or you can email billingteam@horizon-health.org and someone will get back to you within 1-2 business days.

Payment is due at the time of service. Treatment scholarships or payment plans for uninsured individuals are available for those that qualify, based on family size and income. This practice serves all patients regardless of ability to pay.

Please reach out to your health plan to verify your benefit for outpatient mental health or substance use disorder Telehealth services.

Horizon does NOT accept Wellcare, United Commercial or United Medicare.

Can I make a payment on my outstanding balance?

Yes, a credit card payment can be made through our website or by calling the Billing Team at (716) 831-2700. Other forms of payment can be mailed to:

Why do I have different co-payments / co-insurance due?

Co-payments and co-insurance vary by your insurance benefit and the type of service you are receiving. If you have questions, contact your insurance company to review your plan.

Why do I have to pay for my services if I have a Blue Cross Blue Shield (BCBS) plan outside of the Western New York area?

Our contract is with the local BCBS, other BCBS plans may not cover services provided at Horizon. As a courtesy, we will bill your plan and if correct payment is received, we will reimburse you.

Why does my statement show I have a balance due even though I’ve made a payment?

Statements capture payments received through the end of the month. If your payment was received after this date, it will not be reflected until the following month’s statement.
