Supportive Housing Programs

Horizon provides supportive housing services to residents of Erie and Niagara Counties with mental illness and/or chemical dependency who meet federal guidelines for homelessness. Consumers work with case managers to identify and select suitable apartments meeting federal quality and rent subsidy guidelines, and are provided needed assistance with rent payments and securing basic furnishings and fixtures. Additionally, case managers collaborate with consumers’ treatment professionals to encourage and support consumers’ participation in medical and behavioral health treatment, and educational and vocational rehabilitation.
