Michelle Jacob, COTA, CASAC
Corporate Director of Rehabilitative Services

Michelle Jacob came to work at Horizon in 1979, after receiving her associate’s degree in Applied Science from Erie Community College. She started out as a rehabilitation specialist, working with the mentally ill and developmentally disabled. From there, she assumed the position of program manager, responsible for the clinical operations of two of Horizon’s day treatment programs. She became a management team leader and then coordinator of continuing day treatment services. In October 2003, Michelle became senior director for Niagara County Outpatient Services, and was responsible for clinical and business operations for outpatient services in Niagara County and Mental Health Court in Erie County.
Today she is corporate director of rehabilitative services, providing oversight and operational supervision for the Office of Mental Health licenses of Continuing Day Treatment Services, Comprehensive Case Management/Erie County Mental Health Court and Vocational Services.
Michelle is a certified occupational therapy assistant and a credentialed alcoholism and substance abuse counselor.
