How much will it cost?
At Horizon, we are committed to providing the highest quality and most cost-effective care. We accept most commercial health insurance plans, Medicaid and Medicare. Your financial responsibility is determined by your individual contract with your insurer. Our staff will assist you in determining that amount. If paying privately, you may also qualify for a reduced fee based on your income.

What if my insurance isn’t listed?
If you do not see your insurance plan, please contact us at (716) 831-1800 and our Insurance Specialist team will be happy to check your coverage. You may also contact your insurance provider for additional information.
What if I don’t have health insurance?
We can assist you in applying for Medicaid. If it appears that you may be eligible for Medicaid, we will connect you with a Horizon Financial Specialist who will help you begin the application process. Whatever your financial situation, our team is here to assist you with understanding your unique insurance situation and accessing the treatment that you want.
