What to Expect at Your First Appointment
First you’ll meet with one of our excellent staff members to complete and review some basic paperwork. We will also review insurance/payment information and you may also be asked to sign authorizations to obtain your health information from your past and current treatment providers and information from the person or agency that referred you to us.When all of that is taken care of you will meet with a dedicated Counselor, who will speak with you regarding your situation, needs, goals and expectations; and use this information to work with you to develop an individualized recovery and rehabilitation plan that includes selected activities and a physical health screening.

Frequently Asked Questions
If you have health insurance, you will need to bring your insurance card, photo ID, and insurance co-pay (we accept cash, check and most major credit cards).If you do not have health insurance, you will need to bring your photo ID, Social Security Card, proof of address (i.e. current utility bill or postmarked mail), last three pay stubs (if applicable), and cash, check or credit card to pay for your first appointment.
Will I have to do a lot of paperwork?
There is very little paperwork for you to complete. Much of this paperwork occurs during your first visit since counselors are collecting information needed to properly diagnose your condition and develop a treatment plan. We appreciate that people often are seeking immediate relief when they seek care. Be assured that we are doing all we can to reduce the paperwork and begin your treatment as soon as possible.
How will I pay for treatment?
Horizon accepts Medicaid and most commercial health insurances. You will discuss your specific circumstances with our staff at your first visit. Our staff may assist you in applying for Medicaid if you qualify.
