Press Release: New York’s Horizon Health Services Joins Hazelden Betty Ford Patient Care Network
Center City, MN (September 12, 2018) – The Hazelden Betty Ford Foundation today welcomed Horizon Health Services in Buffalo, N.Y., as the newest member of its growing Patient Care Network.
“We are happy to have found a dedicated and experienced leader to join our efforts in the state of New York, where we operate two of our own facilities in Manhattan,” said Bob Poznanovich, vice president of business development for the nonprofit Hazelden Betty Ford Foundation, a national leader in addiction treatment and recovery, headquartered in Minnesota.
“Now more than ever, as we navigate the worst addiction crisis in our nation’s history, there is a need to align quality, like-minded health care providers who are committed to excellent patient care,” Poznanovich said. “The Hazelden Betty Ford Foundation has a long history and culture of knowledge—sharing as a leader in our industry, as does Horizon Health, so it made sense for us to combine our efforts and work together.”
Horizon Health Services is the largest provider of residential and outpatient mental health and substance use treatment in Western New York.
The new collaboration is timely given the nation’s opioid overdose epidemic, and the broader addiction crisis underlying it, which have placed added pressures on individuals, families, communities and health care providers.
“One of the valuable things that Horizon Health Services brings to our national network is deep experience treating people in crisis. They are very good at stabilizing people who have co-occurring substance use and mental health disorders and helping them begin the process of recovery,” Poznanovich said. “We are pleased to have another experienced and leading-edge provider contributing to our network and working with us to address the addiction crisis in New York.”
“Amid the current drug epidemic, we’ve seen a vast increase in demand for crisis intervention and stabilization for our patients and their families,” added Anne Constantino, Horizon’s president and CEO. “Now is the time to come together and work collaboratively. The Hazelden Betty Ford Foundation launched its Patient Care Network — the first of its kind in the addiction treatment industry — in the fall of 2017. Members gain access to tools, resources and collaborative consultation for their organization’s leadership, staff, patients, families and communities – as well as shared learnings and insights from other network members.
“In our area, we have taken the lead in setting out to improve overall health outcomes, and that’s the goal of the Hazelden Betty Ford Patient Care Network as well,” said Constantino. “Together, with a unified focus on quality standards, coordinated care that treats the whole person, and integration with the larger healthcare system, we can improve outcomes and better address the growing needs of our communities. Our goal will always be recovery.”
